Home Care Packages


A Home Care Package is an allocation of funds from the Government to help you purchase services to support your independence.

Peninsula Health has a team of dedicated Case Managers, Care Coordinators and Support Workers who deliver services, advice and expertise to ensure your package is tailored to suit your lifestyle, personal goals and budget.

Caring for you at home

Our team of health professionals and support workers provide expertise to help you stay at home doing the things you love.

We encourage healthy choices that support your physical and emotional wellbeing. Together we will find ways to support the activities that make you happy and healthy.

Your services are tailored to your individuals needs and may include:

  • Case Management to co-ordinate healthcare, attending appointments and other support needs
  • Transport and assistance to go shopping, visit the doctor or participate in social activities
  • Help with cleaning and house maintenance
  • Care from health professionals such as Nursing, Occupational Therapy, Physiotherapy, Diabetes Educator, Exercise Physiologist, Pharmacist or Dentist.
  • Personal care alarms through our provider MePACS
  • Help with personal care such as showering and getting dressed.

Your Case Manager

Our friendly and experienced Case Managers work with you and or your family to design and manage your Home Care Package to make sure it suits your goals and unique requirements.

They will set up services, source useful products, make referrals and monitor your progress and budget. Their job is to make sure your specific care needs are met.

What is a Home Care Package?

A Home Care Package is an allocation of funds from the Government to help you access products and services to support you to live independently at home for as long as possible.

Packages are designed to be flexible to meet your needs today, and in the future.

There are four levels of home care packages available:

  • Level One: people with basic care needs.
  • Level Two: people with low level care needs
  • Level Three: people with intermediate care needs
  • Level Four: people with high level care needs


What services are available in my Home Care Package?

Your Home Care Package can be used for services and equipment that support you to live independently at home, as well as your health and well-being. 

Your Home Care Package service may include:

  • Transport to the shops, appointments or social activities
  • Help with chores – cleaning, laundry and shopping
  • Clinical care – nurses and other health professionals such as physiotherapists
  • Social support – companionship, hobbies and community activities
  • House maintenance – gardening and general upkeep
  • Mobility, independent living and continence aids.
  • Personal care – showering, personal grooming and mobility assistance
  • Personal care alarms – 24/7 monitoring through MePACS


Clinical services within your Home Care Package

At Peninsula Health, our goal is work with you, your family and your health care team, to help you live independently at home for as long as possible.

Your Case Manager will help to identify ways improve your health and wellbeing so you can live your best life.

We can also arrange clinical care as part of your package, to help you with things like:

  • Taking your medicines
  • Helping to monitor and manage health conditions and illnesses such as diabetes and lung disease
  • Podiatry and Physiotherapy
  • Home modifications and assistive equipment
  • Dressing and treating any wounds  you may have
  • Managing incontinence
  • Information about end of life care or care directives.


Understanding your Home Care Package

Your Home Care Package consists of services and fees based on the level of care you require. The budget for each level is set by the Government.

Funding level:

  • Level 1 is $8,044.60 per year or $154.70 per week
  • Level 2 is $14,592.76 per year or $280.63 per week
  • Level 3 is $32,082.96 per year or $616.98 per week
  • Level 4 is $48,772.36 per year or $937.93 per week

Your Home Care Package includes funding for:

Case Management

Your Case Manager will provide support to help you access health and community care by organising referrals, expert advice and advocating on your behalf. We monitor your progress and adjust your plan as your needs change.

The level and cost of case management will vary depending on your health and ability to manage your own healthcare and service needs. Together we consider your needs, lifestyle and budget. We design a detailed plan of support so you can stay in your home, living independently.

Personalised Care Coordination and Support

We coordinate and organise services to suit your ongoing support plan. This includes booking appointments, rostering your support staff, scheduling visits, hiring equipment and managing personal preferences.

Income-Tested Care Fee

Depending on your circumstances you may be asked to contribute a daily fee. This is determined by a Centrelink assessment.

Administration Fee

There will be some administrative fees charged to your package associated with service delivery including insurance, office costs, IT, your monthly statements, reports to government, regular audits, training for staff, health and safety policies and police checks.

How do I know if I’m eligible for a Home Care Package?

My Aged Care, the Government agency responsible for administering the Home Care Package program, will determine whether you are eligible for a home care package.

The My Aged Care assessors will look at your personal circumstances and healthcare needs and work out if you’re eligible and the package level that you’re entitled to.

Our Home Care Package team will support you and your family through the application process to make it as easy as possible for you to access your package.
