Falls Prevention Service

Helping older people maximise safety and minimise the risk of a fall in their daily activities.

The Falls Prevention Service is a specialised assessment service that offers individualised assessment, advice and support for people living in the community who are at a high risk of falling.

What we do

• Comprehensive assessment by a team comprising of a Geriatrician, Physiotherapist, Occupational Therapist and Dietitian to identify the reasons for falls. 
• Individualised care plan with strategies provided to manage risk factors identified.
• Referral to a range of community programs and services for ongoing management or treatment if required 
• Communication with the person’s nominated GP / medical practitioner regarding the care plan for management of the person’s fall risk.

Community groups can access free falls prevention talks by contacting our service.

Who can use the service

People who are aged 16 years and above, and have had:

• Multiple falls in the last year
• One or more falls in the last year, leading to an injury requiring medical attention or a long time spent lying on the floor post fall.

Exclusion criteria and alternate referral options

• People living in aged care facilities
• People who have not had falls in the past 12 months but are unsteady on their feet and have a fear of falling. Please contact ACCESS on 1300 665 781 to identify the best program to suit your needs.

Accessing the service

We accept referrals from all health professionals. Self-referrals from clients and their families and/or carers are also accepted.
