What is a Home Care Package?
A Home Care Package is an allocation of funds from the Commonwealth Government to help you access services and equipment to support you to live independently at home for as long as possible.
Packages are designed to be flexible so it can meet your needs today, and into the future.
There are four levels of home care packages available to suit your care needs – Basic, Low, Intermediate and High. Your needs will be assessed by the Aged Care Assessment Service and a HCP level 1-4 will be approved
What services are available in my Home Care Package?
Your Home Care Package includes care management as well as services and equipment that support your independence at home. Your health and well-being is important to us and we will link you into existing or new activities in the community.
Services in the home are based on your Individual care needs and budget and will be discussed with you when you choose Peninsula Health as your provider. The services may include:
- Transport to the shops, appointments or social activities
- Domestic Assistance – cleaning, laundry and shopping
- Nursing and Allied Health
- Social support – companionship, hobbies and community activities
- Home Maintenance – gardening and general upkeep
- Equipment and aids
- Personal care – showering, personal grooming and mobility assistance
- Personal alarms – 24/7 monitoring through MePACS
- Respite care – residential respite and in-home respite services
Understanding your Home Care Package
Your Home Care Package consists of services and fees based on the level of care you require. The funding for each level is set by the Commonwealth Government and budgeted by your Care Manager which will include care management and package management costs.
How much care management do you need? Peninsula Health divides care management into 2 tiers – basic support and comprehensive support. Please contact our service for more information regarding costs.
Package Management costs include providing administration support to your Home Care Package.
