Our Aged Care Assessment Service (ACAS) ensures frail aged people gain access to the range of Commonwealth Programs that are appropriate to their needs such as care packages and residential aged care services.
What is ACAS?
The Assessment Service (ACAS) is auspiced by Peninsula Health to provide independent assessment services to frail aged persons, Aboriginal and Torres Strait Islanders aged 50 years and above, and to some younger disabled persons.
In addition to the assessment of eligibility for Commonwealth Programs, the ACAS provides information, referrals and coordination with other aged care services, including health and community support services.
ACAS also works collaboratively with other Victorian Aged Care Assessment Services and a wide range of Community Services to identify client issues and provide a common approach to assessment and service development and planning.
Referrals for an assessment are accepted from a range of people including self-referrals, families, carers, GPs and other service providers.
Which locations are covered?
Services are available to City of Frankston, Mornington Peninsula Shire, City of Casey, and Cardinia Shire residents.
