Referrals to CDAMS at Peninsula Health are accepted from General Practitioners (GPs) or Medical Specialists.

About the Cognitive, Dementia and Memory Service

The Cognitive Dementia and Memory Service (CDAMS) is a specialist diagnostic and assessment service for people with memory loss or changes to their thinking. 

What is involved in a CDAMS assessment?

  1.  A CDAMS assessment will include medical and allied health consultations. CDAMS Specialists include clinical nurses; geriatricians; psychogeriatricians; occupational therapists; speech pathologist; social worker; and neuropsychologists

  2. The outcome of the assessments include:
  • expert clinical diagnosis,
  • information on appropriate treatments
  • the development of a care plan
  • education, support and advice
  • direction in future planning including information and advice on Powers of Attorney and driving.
  • appropriate and timely counselling for the person, carer and family to allow greater understanding of dementia
  • linkages for clients or their family to other service providers or community supports
  • Facilitate cross referrals to other service providers including:
    1. Aged Care Assessment Service (ACAS)
    2. Aged Persons Mental Health Team (APMHT)
    3. Community based services
    4. Dementia Australia
    5. Occupational Therapist Driving Assessments

Clinics are held at both The Mornington Centre and Rosebud Community Rehabilitation Centre.
