About the service
The goal of our MI Health program is to improve and maintain the health and wellbeing of people who are homeless or people living in insecure accommodation. The Mobile Integrated Health team will visit you where you live and help you get services to keep you well. We also offer Aboriginal and Torres Strait Islander Access and Support services. Our team is comprised of experienced social workers, nurses and community development workers.
This service supports people who are:
- homeless or at risk of being homeless
- in insecure housing such as rooming houses, caravan parks, pension level Supported Residential Services (SRS)
- facing eviction in private or public rentals
- People who have complex health problems who need support to remain engaged with services
- Ability to fund eligible equipment to support living independently at home
- Financial hardship effecting housing security or finding more suitable housing
- Ability to fund goods to help you access housing, employment and training
- Support you to access legal services and respond to debt
Who can use the service?
This service is suitable for people over the age of 65 years OR younger people with a disability and/or complex health needs who are:
- not linked into appropriate services and are homeless OR
- are at risk of homelessness
