Informed decision‑making for better, safer care
CareSync Exchange is a secure electronic system implemented by Victoria’s Department of Health. It provides clinicians with centralised and up‑to‑date patient health information sourced from Victorian public hospitals and health services.
CareSync Exchange allows Bayside Health Peninsula and other participating Victorian public hospitals and health services to securely share relevant health information to support your care and treatment in Victoria.
Using CareSync Exchange, health professionals at Bayside Health Peninsula can view information from your visits to other Victorian public health services. Similarly, clinicians at other participating public health services can access relevant information from Bayside Health Peninsula if you attend their service, to help support safe and coordinated care.
CareSync Exchange and My Health Record are separate but complementary systems that support your care.
- CareSync Exchange is a Victorian Government–operated system that shares health information between public hospitals and health services only, to support care at the point of treatment.
- My Health Record is a national system operated by the Australian Government and may include information from public and private healthcare providers, such as GPs, community pharmacies and private hospitals.
CareSync Exchange operates under the Health Services Act 1988, which permits the sharing of health information between Victorian public health services to support patient care. My Health Record operates under separate Commonwealth legislation and includes opt‑out arrangements.
CareSync Exchange supports:
- More informed decision-making about patient care
- Less need for paper records, phone calls, or faxes between health services
- Reduced burden on patients to remember or repeat their medical history
- Better continuity of care, especially if a patient attends multiple hospitals or health services
Authorised clinicians may access:
- Patient details, including demographics and emergency contacts
- Patient hospital visits including inpatient, emergency, and outpatient
- Clinical documents, diagnostic test reports and hospital discharge summaries
As CareSync Exchange continues to evolve, clinicians will be able to view additional clinical information, such as:
- medications
- allergies
- radiology reports
CareSync Exchange is designed to protect patient privacy and security.
Only authorised doctors, nurses and other healthcare professionals involved in your care can access your information. Strong safeguards apply under the Health Services Act 1988, and serious penalties exist for misuse of health information.
CareSync Exchange operates on a private and secure network that can only be accessed by participating Victorian public health services. Additional protections, including encryption, firewalls and monitoring systems, help prevent unauthorised access.
Patients may request a User Activity Report that shows access to your information in CareSync Exchange. The report includes:
- The role of the user who accessed the information (for example, doctor or nurse)
- The date and time of access
- The type of activity performed (e.g. viewing a test result)
- Any additional relevant details.
The report will include access that has occurred across all participating Victorian public health services.
The report does not display clinician names, but does include clinician role type, the date and time of access, and the activity performed.
To request a User Activity Report, please email
hisfrankston@phcn.vic.gov.au and include:
- A clear request for your User Activity Report
- The date range you would like the report to cover
- A copy of your photo identification showing your full name (given name and surname) and date of birth, to confirm your identity. If your photo ID does not show these details, you may also provide a Medicare card.
