At Home Care: Upgrading Hip and Knee Replacement Recovery

Peninsula Health’s new approach to hip and knee replacement recovery is transforming patient experiences across Frankston and the Mornington Peninsula.

Under the At Home initiative, which aims to keep people out of hospital, and reduce the reliance on bed-based services, Peninsula Health now provides a supportive environment that extends beyond the confines of the hospital.

Here’s how Peninsula Health has enhanced their service:

  • Streamlined Recovery Pathways and Enhanced Patient Support
    Patients undergoing hip and knee replacements now benefit from a clear recovery pathway, with the flexibility to scale up support for higher needs. Every patient automatically receives follow-up care and communication from the orthopaedics rehabilitation team, eliminating waitlists and ensuring consistency at discharge. This approach contrasts with the past, when patients were sometimes sent home without any support. By informing patients about their care pathway before their hospital stay, we manage patient expectations, reduce any fears, and enhance patient outcomes. Early and consistent support empowers patients, sets clear expectations, and fosters a positive outlook on their surgical and rehabilitation journey
  • Group-Based Rehabilitation: A significant component of our service involves group-based care in multiple locations close to people’s homes. Patients participate in rehabilitation exercises and learn essential skills to manage at home. These group settings allow patients to mix with others who have undergone joint replacements, fostering a supportive environment while receiving care tailored to their personal requirements
  • Social Benefits and Community Integration: Group-based care offers substantial social benefits, as patients engage with peers in multiple locations close to their homes. This proximity and community aspect alleviate the isolation often associated with recovery, promoting a sense of camaraderie and mutual support
  • Rehabilitation at Home: Particularly for older patients, rehabbing at home is more advantageous than traditional rehabilitation centres. At home rehabilitation not only provides comfort but also enhances recovery outcomes, by enabling patients to perform daily activities in their own environment, making the transition back to normal life smoother
  • Telehealth Integration: Telehealth and video clinics provide seamless access to the orthopaedics rehabilitation team, minimising hospital visits and improving the patient experience. The new service incorporates remote clinician appointments via Telehealth, minimising the need for more hospital visits. Whether through video or phone calls, patients can receive timely check-ups and consultations from the comfort of their home
