At Home is our leading service to move additional medical care into the home and community environment, where we aim to treat people in their homes and make home the first preference for patients to receive care.
We work as a team where our clinicians coordinate your care and partner with you to ensure our care is based on what is important to you. Home care offers our patients the same level of service instead of going to hospital, or once a patient is in hospital, to reduce the time they spend there.
Services available for you At Home
There is treatment available to manage acute issues, as well as long-term health conditions.

- Intravenous treatments
- Palliative care
- Advance care planning
- Wound & drain tube management
- Clinical monitoring
- Nursing, allied health, and personal/respite care
- Rehabilitation and functional improvement
- Complex and chronic disease management with allied health (Physiotherapy, Occupational Therapists, Social Work, Dietetics, Speech Therapy, Podiatry, Pharmacy)
- Nursing and medical support to residential care facilities and the GP’s who service them
- Health service-employed doctors to liaise with GP where needed
We can provide services within quick timeframes to prevent the need for you to go to a hospital or as an alternative to a hospital. You can also access our services to help keep you well. work with you to achieve your treatment goals and coordinate any allied health community services you may require.
How services are delivered in the home
Depending on your treatment needs, services can be provided 7 days a week, with up to 2 daily visits by doctors, nurses, and personal/respite care staff, between 8.00am to 9.00pm. Allied health community services are provided Monday to Friday. The services you receive are based on your individual needs.
The duration of your care is unique to you and can be provided by a variety of Peninsula Health staff. Some patients require a few days of care, others need longer-term care organised.
Home does not reflect the sole location of where care will be provided. Rather, ‘home’ is a broad term to encompass a setting outside a hospital bed where care is provided across settings depending on patient needs. These settings could include a person’s home, a Peninsula Health centre, a community venue, a group class, or virtually (face time/telephone call).
