Documents & Referees

What information and documentation are required for my applications?

For your application to PMCV please refer directly to the Candidate Handbook available from the PMCV website The Match

As part of your application to Bayside Health Peninsula, you’ll need to provide:

  • A resume (maximum three pages, must include PMCV number and contact details for two referees)
  • Your academic transcript (must include ‘Grading Key’ from your education provider)
  • 2x clinical placement reports (must include formative & summative reports for each placement)
  • Optional – cover letter

Refer to the Bayside Health Peninsula Application Guidelines for further information.

How recent do my referees need to be?

Referees are ideally from your most recent placements. Placements within the last 12 months are preferred; however we understand some applicants may have varied study pathways and older placements may be considered.

Can my referees be from different health services or specialties?

Yes. Referees may be from different hospitals or clinical areas, as long as they can independently comment on your clinical performance.  We ask for 2 referees from 2 different placements; these can be from the same Health Care Service.

Do my referees submitted to PMCV also need to be listed on my resume?

Yes, your listed referees submitted to PMCV must match those provided on your resume. Details must include their name, profession, Health Service they worked for during your placement, placement dates, contact phone number and email address.

Can I supply work references?

No, we do not accept work references.  References must be from 2 different student placements.  If you cannot supply a 2nd referee from a student placement, a reference may be supplied by either a Nurse/Midwife manager, educator or clinical supervisor for a RUSON/RUSOM role.

Do my referees need to match the stream I am applying for?

Not necessarily for General or Mental Health streams.  Relevant experience is taken into account, but using referees from another specialty will not disadvantage your application.

For Midwifery stream 1 reference must be provided from a midwifery placement.

Should I include my future placements when detailing my placement on my resume?

Yes, if you are aware of any scheduled future placements, please detail this information in your resume.

Do I need to provide previous academic transcripts if I was an EN?

Applicants need to provide and upload an official or downloadable PDF transcript with any academic transcripts that have contributed RPL or credit points towards the Nursing or Midwifery degree. Applicants must also submit any additional academic transcript keys associated with their RPL or credited units.

Can I submit 1 formative and 1 summative assessment clinical placement document that count as 2 clinical placement reports?

No. Two complete clinical placement reports are required.

Reports must be from two different placements and each report must include:

  • Both the formative and summative assessment
  • Written comments

If your placement was split across two health services (for example, formative completed at one service and summative at another), this is considered as one clinical placement report, not two.

You do not need to provide us with any skills assessments or additional placement competencies.
