Peninsula Health’s At Home monitoring service continues to transform healthcare within the comfort of people’s homes, thereby preventing avoidable hospital admissions.
Since the service began, there has been a total of 194 patients monitored across Frankston and the Mornington Peninsula, with an average growth rate of 13 new patients accepted by the service each month.

Currently this service sees patients with heart failure, and respiratory conditions whose conditions can lead to hospital presentations. The Chronic Disease Monitoring team manages the clinical matters, and the MEPACS team provides assistance with technicalities, such as patients requiring check-in assistance.
