Young Adults Diabetes Service (YADS)

This clinic sees young people between the ages of 16 to 25 years. It is a multidisciplinary clinic with Endocrinologists, Diabetes Educators, Dietitians and a Clinical Psychologist .

Referrals

Referral addressed to named head of clinic is preferred. 

The GP Referral Template located within the Mastercare Referralnet system is the preferred referral tool.

– Diabetes and severe vomiting
In Business Hours call Diabetes Educator PH 9784 7625 to arrange urgent appointment for some new Type 1 diagnoses to avoid ED, but otherwise via ED.

Eligibility criteria

  • Young People aged 16-25 years Type 1 or type 2 diabetes

Exclusion Criteria

  • < 16 years of age Paediatric Diabetes Clinic
  • > 25 years of age Adult Diabetes Clinic
