About Peninsula Health’s Community Rehabilitation Program

The Community Rehabilitation Program provides short-term rehabilitation to people recovering from an illness, injury, surgery and/or hospitalisation. The program comprises both Home Based Services and Centre Based Services.

Centre based services are provided at the Frankston Community Rehabilitation Centre and Rosebud Community Rehabilitation Centre.

Some clients may receive a combination of Home and Centre Based services and will remain with the same clinician. All clients receive the contact details of their clinician.


Early referrals are appreciated and can be made via ACCESS using the ACCESS Referral Form. Please specify the urgency when making a referral and include all past history. Attaching a patient summary to the referral is much appreciated.

Referrals are reviewed by a senior clinician, and are triaged into three categories:

  • Category 1 (admission expected within 5 days). E.g. – Post discharge, joint replacement
  • Category 2 (admission expected within 15 days). E.g. – Chronic pain
  • Category 3 (admission expected within 90 days). E.g. – Weight loss

Clients who have an active claim through a compensable body (eg: TAC or Workcover) should first be referred to a private provider. If CRP is the most appropriate provider, approval for the service and details of the claim should be sought from the client’s case manager prior to referral to CRP.

GPs are contacted when the client is admitted and discharged.  Patients may be discharged from one discipline but continue seeing another, so discharge recommendations are sent at discharge from each discipline. CRP clinicians are happy to provide copies of care plans if required.

Home Based Services

Rehabilitation clients referred for Home Based Services should meet any of the following criteria:

  1. The client needs to be in their own environment to learn new skills that will improve participation in their daily living
  2. The client will have difficulty transferring skills they learnt in the centre/hospital into their own environment
  3. The client needs to practice skills (communication, cognitive, mobility) in their local community
  4. The client will have difficulty coming to the centre due to:
    • Surgical restriction or pain
    • Lack of transport due to expense or geographical isolation (when all other methods of transport to the centre have been discounted)
    • Fatigue that will impact on participating in rehabilitation to their full potential.
