Advanced Care Planning Service

About the service

The aim of this service to provide assistance to clients who wish to discuss and record their preferences for future care in the event that they lose the capacity to make or communicate decisions because of serious illness, injury or disease.

This is a free service.

Who can use the service?

This service is suitable for people who:

  • Are aged 18 years or over who require assistance/support with advanced care planning.
  • Have a GP/Medical Specialist or be willing to visit a GP.
  • Have the capacity to make their own health care decisions or have a ‘person responsible’.

How to access this service

Telephone 1300 665 781 between 8:30am-5:00pm (Monday-Friday) and an Access Worker will give you further details on the service.
