Our Youth Community Mental Health Team (YCMHT) can support you Monday to Friday, 8:30am – 5:00pm. Our team is staffed by mental health professionals including mental health nurses, social workers, occupational therapists, psychologists, doctors and a psychiatrist.
We can provide you with specialist assessment and treatment for significant mental health difficulties and other concerns that may impact on your capacity to function and engage in life.
Our interventions include:
- Clinically informed case management and outreach
- Specialised psychological treatments
- Supportive counselling
- Medication
- Family work
Every service we provide has a strong focus on psychosocial recovery (eg. life goals important to the young person), is trauma informed and inclusive of diversity.
Our YCMHT can meet with you weekly though the frequency and type of support can be changed at times of increased need. Our Crisis Assessment and Treatment Team (CATT) can provide you support after hours if required.
Who we care for
If you are aged between 16-25 years our YCMHT can support you and work with your family, carers and supporters and the other services you may be connected too.
You may be referred to our service following a mental health crisis or you may have had challenges with your mental health and functioning for some time.
A referral to Peninsula Health Mental Health Service should be considered if your mental health difficulties and concerns:
- have been persistent
- are impacting many aspects of your life (ie. work, education, relationships)
- are affecting your safety and/or the safety of others
- have not responded to treatment and support provided by another professional or service
How to access this service
All new referrals for our YCMHT are made via Mental Health Telephone Triage.
You can self-refer or somebody close to you can make a referral on your behalf. We will always want to talk with you before offering support and making any plans.
If you are already accessing a Peninsula Health Mental Health and Wellbeing Service (for example a crisis or inpatient service) an internal referral can be made on your behalf.
