Transforming our Mental Health & Wellbeing Service
As of 20 November 2023, Peninsula Health’s Youth Community Mental Health Team (YCMHT) will be known as the Integrated Youth Assertive Treatment Team (IYAT). Learn more about how we are transforming our service.
Our Integrated Youth Assertive Treatment Team (IYAT) can support you Monday to Friday, 8:30am – 5:00pm.
Our team is staffed by mental health professionals including mental health nurses, social workers, occupational therapists, psychologists, wellbeing workers, peer workers, doctors and a psychiatrist. Our Consumer and Family Carer Peers provide valuable support from a lived-experience perspective.
We can provide you with specialist assessment, treatment and care for significant mental health difficulties and other concerns that may impact on your capacity to function and engage in life.
Our interventions include:
- Clinically informed case management and outreach
- Psychological treatments
- Supportive counselling
- Family work
- Wellbeing supports and assistance with phsycial health
- Liaison with other services and referral
Every service we provide has a strong focus on psychosocial recovery (eg. life goals important to the young person), is trauma informed and inclusive of diversity.
Our IYAT can meet with you once per week, though the frequency and type of support can be changed at times of increased need. Our Crisis Assessment and Treatment Team (CATT) can provide people aged 16 years and over, with support after hours and on weekends if needed.
Who we care for
If you’re 15-25 years old, our Integrated Youth Assertive Treatment Team can support you, and work with your family, carers and supporters. We also work in partnership with other health services you may use. You may be referred to us if:
- your mental health and/or Alcohol and Other Drugs (AOD) difficulties have been persistent
- your mental health is affecting your safety and/or the safety of othersyour mental health is impacting many aspects of your life (ie. work, education, relationships) your mental health has now not improved following treatment and support provided by other professionals or services after a mental health crisis
How to access this service
To access support from our Youth Mental Health Team, call Mental Health Telphone Triage on 1300 792 977. You can get help for yourself, or someone you trust can call for you. If you’re a carer, you can call for someone you’re supporting,
We will always want to talk with you before offering support and making any plans.
If you are already accessing another Peninsula Health Mental Health and Wellbeing Service (for example a crisis or inpatient service) they can refer you to us.
Appointments may be carried out in a variety of methods, including clinic based, Telehealth, and/or home visits.
The IYAT Team is located at two convenient locations across the Mornington Peninsula.
