Our Integrated Youth Assertive Treatment Team (IYAT) can support young people between the ages of 12-25, Monday to Friday, 8:30am – 5:00pm.
Our team is staffed by mental health professionals including mental health nurses, social workers, occupational therapists, psychologists, wellbeing workers, peer workers, doctors and a psychiatrist.
We can provide you with specialist assessment, treatment and care for significant mental health difficulties and other concerns that may impact on a young persons capacity to function and engage in life.
Our interventions include:
- Clinically informed case management and outreach
- Psychological treatments
- Supportive counselling
- Family work
- Medication
- Wellbeing supports and assistance with physical health
- Liaison with other services and referral
Every service we provide has a strong focus on psychosocial recovery (e.g. life goals important to the young person), is trauma informed and inclusive of diversity.
Our IYAT can meet with you once per week, though the frequency and type of support can be changed at times of increased need. Our Crisis Assessment and Treatment Team (CATT) can provide young people with support where required and indicated after hours and on weekends.
Who we care for
The Integrated Youth Assertive Treatment (IYAT) Team works with young people aged 12-25 years, their families, carers and supporters who require a period of extended care from 3 – 12 months. If you’re 12-25 years old, our IYAT team can support you, and work with your family, carers and supporters. We also work in partnership with other health services you may use. You may be referred to us if:
- your mental health and/or Alcohol and Other Drugs (AOD) difficulties have been persistent
- your mental health is affecting your safety and/or the safety of others
- your mental health is impacting many aspects of your life (ie. work, education, relationships)
- your mental health has now not improved following treatment and support provided by other professionals or services after a mental health crisis
How to access this service
To access support from our Youth Mental Health Team, call Mental Health Telephone Triage on 1300 792 977. You can get help for yourself, or someone you trust can call for you. If you’re a carer, you can call for someone you’re supporting,
We will always want to talk with you before offering support and making any plans.
If you are already accessing another Peninsula Health Mental Health and Wellbeing Service (for example a crisis or inpatient service) they can refer you to us.
Appointments may be carried out in a variety of methods, including clinic based, Telehealth, and/or home visits.
