Hospital Outreach Post-Suicidal Engagement (HOPE) Service

HOPE is a community-based outreach program for people who have attempted suicide or been identified as at-risk of suicide from the age of 18.

Our HOPE Service operates from the Tarnuck Centre in Frankston between the hours of 8:30am – 5:00pm Wednesday to Monday, with extended hours on Tuesday from 8:30am until 8:00pm. We can also arrange to see you in your home if that is more helpful to you.

Our team of experienced mental health professionals provide a responsive service that is uniquely designed to meet your needs with a focus on reducing the risk of suicide and self-injury in the future. Our HOPE Service works with you on identifying and managing triggers for suicidality and planning for safety.

The Service uses therapy-based approaches and can also provide support to the persons’ carer or family though the carer lived experience role. We also have access to a psychiatrist in HOPE and can initiate or change medication if that is an important part of your care.

Our HOPE Service will work with you, your support network and other treating professionals to ensure you have the help you need to stay safe and recover from illness on finishing with the service.  

Who we care for 

Our HOPE Service works with people 18 years and older who have attended the Emergency Department in suicidal crisis or after a suicide attempt. Referrals may also be made by our Crisis Assessment and Treatment Team (CATT) for people who have been referred with suicidal crisis or after a suicide attempt.

To access Our HOPE Service will you need to be open to meet about weekly for a 3 month period to work on your safety and recovery. If you require more assertive or comprehensive specialist mental health support a referral will be made to an alternative team within the Peninsula Health Mental Health and Wellbeing Services better able to provide the support you need.  

How to access the HOPE Service  

The HOPE Service only receives referrals from within the Peninsula Health Mental Health and Wellbeing Service.
