Aged Persons Community Mental Health Program (APCMHP)

Our APCMHP provides specialist community-based mental health treatment for people aged 65 years or over and/or those with a formal diagnosis of neurocognitive disorder. Our APCMHP will work with you to provide person-centred and recovery-oriented care. Our APCMHP works in partnership with the Aged Inpatient Unit (1WA) and our psychogeriatric nursing home (Carinya). 

Our Team is staffed by:

  • consumer peer workers

We have three different community teams that can provide you with support:

Aged Persons Intensive Community Assessment Treatment Team (APICATT) 

Our APICATT provides you with face-to-face assessment and treatment for during an acute stage of illness. We can see you at home or at one of our clinic sites 7 days a week between the hours of 8:30am – 5:00pm.

Case Management   

Our Case Management Service provides coordination of care if you need medium to longer term support to achieve your recovery goals whilst living with mental illness in the community and residential care settings.  Our Case Management Service is available Monday – Friday, 8:30am – 5:00pm.

Residential Support Program (RSP

Our RSP can provide your residential care setting with advice and support to your health care needs including helpful ways to support you or those your care about with behavioural and psychological symptoms of dementia or other severe neurocognitive disorders. Our RSP is available to people Monday – Friday,  8:30am – 5:00pm

Who we care for

Our APCMHP provides specialist community-based mental health treatment for people aged 65 years or over and/or those with a formal diagnosis of neurocognitive disorder living in the Frankston and Mornington Peninsula Area. We can provide support for you from home or from a residential care facility.

How to access this service

All new referrals can be made through our Mental Health Telephone Triage service on 1300 792 977. The majority of our referrals for the APCMHP come from general practitioners, family, self-referral or 1 West.
