The Response, Assessment and Discharge (RAD) team was established to assess patients whose needs could be more appropriately met through non-hospital services such as Peninsula Post-Acute Care, referral to Complex Care Program, community supports or respite care in a supported residential facility.
The team provides a holistic trans-disciplinary assessment for appropriate patients presenting to the Emergency Department. The team consists of Social Work, Physiotherapy, Occupational Therapy and Nursing. This team provides a key link between the Emergency Department, the community and the hospital wards.
Should a patient not require admission to the hospital the RAD team can assist in arranging the necessary services to maintain them in their home. An outreach service is also available for suitable patients referred by Ambulance Victoria. The aim is to meet patient’s needs within their own environment and avoiding unnecessary presentation to the Emergency Department.
Locations
The RAD Team operates in the Frankston Emergency Department and the Rosebud Emergency Department.
