This service is for people who need to exercise and make changes to their lifestyle and to help prevent and manage long term health conditions.
Your exercise physiologist will talk with you about your condition in order to provide an exercise program that is safe for you to follow and will help you to achieve your goals.
They will show you how to exercise safely, review your progress regularly and update your program as you need. Your exercise physiologist will also help you learn how to manage your condition and exercise independently for the long term.

The service is available across our Community Health sites at Frankston, Rosebud, Mornington and Hastings.
How do I access this service?
Call 1300 665 781 between 8:30am-5:00pm (Monday-Friday) and an ACCESS worker will give you further details on the service.
