Helping people understand their heart condition and live active lives.
The Healthy Heart Program is an outpatient cardiac rehabilitation program that provides support and education to people recovering from a heart event. The program encourages lifestyle changes that aim to reduce the risk of further heart problems.
The Healthy Heart Program offers and eight week group program consisting of supervised exercise and heart information sessions. Groups are held at Frankston Community Health, Rosebud Community Health and Hastings Community Health.
Individual support can also be arranged for those than cannot attend.
Who can use the service?
You are recommended to attend the Healthy Heart Program if you:
- Have experienced a cardiac event (such as a heart attack, heart procedure or surgery),
- Are diagnosed with heart failure
- Require assistance to manage risk factors for heart disease

$10.30 for initial assessment
$6 per group session
