Service information
With more than 120 social workers playing a critical role in intervention, support and care, Peninsula Health’s social work department works across multiple programs and sites to assist patients, their families and carers with a range of issues.
Our social workers undertake the following activities:
- comprehensive psychosocial assessments
- intervention and rigorous discharge plans, in consultation with the patient, their family, carers and significant others
- risk assessments
- counselling and advocacy support
- crisis intervention
- the provision of specialist clinical expertise, in assessing the psychosocial needs of individuals in a number of health settings
Adjusting to health issues can be a stressful time for patients and families. We provide support, education and counselling for patients, families and carers. Social Workers provide assistance for a range of issues, including but not limited to:
- Adjustment to illness or disability
- Reactions to loss and grief
- Bereavement support
- Maintaining relationships with family
- Trauma
- Managing feelings of anxiety, depression and loneliness
- Carer stress
- Family violence
- Child/elder abuse
Locations
Social workers are based across the acute, sub-acute and community settings within Peninsula Health (Frankston Hospital, Golf Links Road Rehabilitation Unit, Golf Links Road Palliative Care Unit, The Mornington Centre and Rosebud Hospital).
How can I access this service?
This service can be accessed by inpatients and patients who have been referred internally.
For students
The Social Work Department provides opportunities for clinical placements for social work students. We have formal arrangements with Monash University, RMIT University, Deakin University and Melbourne University. Student placements are to be arranged through University Fieldwork Departments. Our Student Unit provides an average of 24 placements per year. Our placement duration for both first and second placements is a minimum of 500 hours.
