The new At Home lymphoedema service aims to provide more cohesive treatments for patients, allowing people to stay well and remain at home as much as possible throughout their care journey while receiving the care they need.
Peninsula Health recognised the need for a local service with a person-centred approach and has transformed its lymphoedema service through the At Home initiative. Here’s how it’s making a difference:
Expanded Service Reach: People across Frankston and the Mornington Peninsula now have access to a comprehensive lymphoedema service, ensuring continuity of care within their local community.
Specialised Expertise: The new service has bolstered its health professional team with additional lymphoedema expertise. A collaborative approach, led by a central lymphoedema therapist, ensures that everyone receives personalised care tailored to their unique needs
Active participation: Early diagnosis is key to managing lymphoedema effectively. Patients are equipped with valuable information on self-management techniques, empowering them to monitor their condition at home and actively participate in their treatment journey
Prevention Education: Greater prevention strategies and lymphoedema prevention education for patients
Telehealth Integration: The new service incorporates remote clinician appointments via Telehealth, minimising the need for more hospital visits. Whether through video or phone calls, patients can receive timely checkups and consultations from the comfort of their home
Personalised Care Plans: Each patient receives a customised prevention and treatment plan, tailored to address their specific needs and challenges
