South East COVID Pathway
(03) 9119 1451
The South East COVID Pathway is operated by Sandringham Ambulatory and Community Care (SACC) in partnership with the South East Metro Health Service Partnership (SEMHSP). The pathway was developed to support GPs in managing COVID-19 positive patients in the community and provides advice and streamlined escalation of care of patients at risk and in the event of deterioration. SACC provide patient monitoring and will handover vulnerable patients with ongoing concerns needing follow up and leaving the program, to GPs.
Clinical Advice and Escalation of Care
(03) 9788 1700
The Community Care team will remain available for GPs to discuss clinical care of patients. This is especially the case when there are clinical dilemmas, when a patient is likely to need admission or definitely needs remdesivir (eg. transplant patient, active chemotherapy patient).
Community Care is open 8am – 9pm 7 days a week
