Is hospital care at home new for Peninsula Health?
No, it’s been available for over 30 years. However, its benefits are now more widely recognised, with growing research showing it often leads to better outcomes. Peninsula Health is innovating in the at home hospital care space, with more treatments becoming available to patients in the home.
What are the benefits of at home medical care?
At-home medical care allows you to recover in the familiarity & comfort of your own home, surrounded by loved ones. You avoid travel, parking costs, hospital waiting times, and the restrictions of hospital visiting hours. This option offers more freedom and independence, helping many people recover faster. It’s also safe, with hospital support available if your condition changes.
Is it safe to receive hospital care at home?
Yes, receiving hospital care at home is safe. Peninsula Health adheres to strict policies, procedures, and standards to ensure safety, whether care is provided in the hospital or in the home. Regular audits are conducted, and all necessary medical equipment and expertise is provided based on your individual needs.
Is receiving hospital care at home considered a continuation of hospital care or a discharge from Peninsula Health?
Some At Home services are considered a continuation of your hospital care, meaning it is not a discharge. In these cases, your care will be coordinated seamlessly, with hospital staff and the At Home service meeting all your medical, prescription, and referral needs during this time. However, other At Home services are not classified as hospital care, so you should continue to visit your GP. You will be informed whether your care is considered a continuation of hospital care.
Is my GP involved with my care at home?
With some At Home services, you remain an admitted patient, so the hospital provides all your medical care during that time. A letter will be sent to your GP to inform them of your admission to our service, and a copy of your discharge summary will be sent when you’re discharged. This transfers your care back to your GP, so please ensure we have the correct GP information on file.
Are there any extra expenses I could incur when receiving hospital care at home?
No, there are no extra costs where you are still an admitted patient of the hospital. You’ll receive the same professional healthcare team, equipment, and resources you’d expect from high-quality hospital care.
There may be some small out-of-pocket costs for services where you are not considered to be an inpatient. You will be informed of any up-front costs.
Can I get help with chores like showering, meals, and cleaning while I recover?
Yes. We can arrange services to assist with mobility, personal care, shopping, meals, domestic tasks, and respite. These services are provided based on individual needs and are available if no other help is available through family or friends.
What information will I receive about the care that my significant other will get at home, including my role and responsibilities?
Loved ones play a crucial role in a person’s recovery. As a caregiver, you’ll receive guidance and support from the healthcare team. They will address any concerns you have and provide you with the necessary information and practical support to help care for your loved one at home.
Can I speak with the healthcare team before my significant other comes home, especially if I have concerns?
Yes. You will be consulted at the start and throughout the recovery process. You’ll receive information on how to care for your loved one and be offered the support you need. You’ll also be informed about your role and asked to consent to the care plan. You are welcome to discuss any concerns at any time.
