Pharmacist wins award for championing person centred care

Pharmacist Grace Loh has been recognised by her peers for going above and beyond to provide a positive experience for patients.

She is Peninsula Health’s staff nominated Patient Experience Champion for 2018. The award is part of Peninsula Health’s celebration of Patient Experience Week.

Grace, who has recently started working as a pharmacist in mental health, says the best part of her job is interacting with patients.

According to her colleagues, Grace always has the patients’ best interests at heart in everything she does.

“She demonstrates care and compassion to everyone she deals with every single day and does so without any judgement and always with a smile.”

“I really like talking to people about the medications that they are on, their priorities for medications and making sure that they understand why changes may have been made to their medication while they are in hospital,” she says.

“We encourage patients to be involved in discussions about medications and to report back on how they feel the medications are for them. It’s really important that if patients are having side effects, that they discuss that with their prescriber.”

The other highlight of Grace’s job is the people she works with.

“I love working with an amazing group of people that really are patient centred.”

After eight years at Peninsula Health, one of the highlights of her career is from when she first started.

“Back when I was a pretty new pharmacist, I received a poem from a patient which meant a lot to me.”

“It’s really nice to feel that patients appreciate what you do.”
