Research is an integral part of the Department’s culture as it underpins the aim to provide high quality evidence-based intensive care. The last decade has seen substantial progress in critical care-related research generated from the Department of Intensive Care.
The establishment of a national database has provided an invaluable tool for research activities. This database has been in existence in various forms since 1990 when the decision was made by Australian and New Zealand Intensive Care Society (ANZICS) to collect national information about ICU resource allocation, patient demographics and patient outcome. The national database known as AORTIC (Australian Outcomes Research Tool for Intensive Care) stores raw data and undertakes calculations to provide the Acute Physiology, Age, & Chronic Health Evaluation (APACHE III) and Simplified Acute Physiology (SAPS) score for each patient. Metropolitan and Regional Intensive Care Units generate routine reports from their own in house databases and submit these electronically on a regular basis to the national database run by ANZICS.
Research interests at Frankston Intensive Care Unit are varied and include sedation, renal replacement therapy, nutrition, echocardiography and ultrasound, extracorporeal techniques mechanical ventilation and sepsis. The department has been a member of the ANZICS Clinical Trials Group (CTG) for many years and has collaborated on numerous multi-center trials. Links with industry have provided grants for further trials.
Collaboration with Fisher Paykel has created opportunities for evaluating methods of oxygen delivery and this data has led to international presentations and publications in peer reviewed journals.
Associate Professor Ravi Tiruvoipati is the Director of ICU Research and is PhD candidate with Monash University and is investigating the role of C02 in mechanically ventilated patients. He has extensive experience in ECMO and was one of the lead investigators of the landmark CESAR study published in the Lancet. Prof Botha is a past member of The Sedation and Analgesia Advisory Board in Intensive Care (SAABIC) and multi centre research in ICU delirium and Sedation was completed with this group. He has been site investigator of numerous CTG trials and was on the Management Committee of the NHMRC funded Nephroprotect Trial. Dr Kavi Haji is a PhD candidate with Melbourne University and her thesis is on the role of bedside echocardiography and ultrasound on management of the critically ill. The Research Co-ordinator is Mr Cameron Green who has an MSc from the University of Edinburgh for his work on Delirium.

A randomised controlled trial investigating the use of Angiotensin-II for patients with catecholamine resistant shock
A multi-centre randomised controlled double-blind trial investigating the impact of energy-dense enteral nutrition on the outcomes of critically ill patients, when compared to standard enteral nutrition.
Nebulised Heparin Trial
A multi-centre randomised controlled trial of nebulised heparin for the prevention of lung injury in mechanically ventilated patients.
An investigation of the role of induced Hyperthermia in the treatment of critically ill patients with sepsis.
Investigation of the use of Haemolung Respiratory Assist System (RAS) for management of severe hypercapnia among mechanically-ventilated critically ill patients
Evaluation of respiratory and cardiac Ultrasound in predicting outcomes of weaning from mechanical ventilation. The aim of the study is to evaluate the use of Ultrasound in predicting outcomes of weaning from MV by analyzing diaphragmatic movement, systolic and diastolic Left Ventricle function, and detecting worsening or newly developing pulmonary oedema.

DELAYED DISCHARGE STUDY
This study compares differences in outcomes between patients who have their discharge from ICU delayed with those who have no delay in discharge
A multi-centre randomised controlled trial investigating the impact of transfusing the freshest-available Red Blood Cells (RBC), when compared to standard practice (transfusing the oldest-available RBC), on patient outcomes.
A randomised controlled trial comparing two modes of spontaneous supported ventilation, Pressure Support Ventilation (PSV) to Proportional Assist Ventilation (PAV+), on patient outcomes, including length of mechanical ventilation and ICU length of stay.
Impact of Delay in Rapid Response Call activation on Patient Outcomes
This study aimed to investigate the impact of delay in activation of a rapid response call on patient outcomes including unplanned ICU admission and in-hospital mortality
This study aimed to develop and validate the first ever scale for the assessment and monitoring of pressure injuries to the mouth and lips.
Sleep in the ICU – Pilot Study
An investigation of the self-reported quality of sleep among non-mechanically ventilated patients admitted to the Intensive Care Unit.
This multi-center will evaluate the effect of Goal Directed Therapy on septic patients admitted to the Emergency Department.
Impact of routine transthoracic echocardiography and respiratory ultrasound assessment in critical care. The study aims to evaluate the role of routine focused transthoracic echocardiography and respiratory ultrasound in the assessment of critically ill patients as extended clinical examination.
Ultrasound assessment of regional diaphragmatic movement and the correlation with subphrenic organ displacement in critically ill patients. The aims of the study are to evaluate the correlation of movement between parts of the hemidiaphragm, assess the correlation between liver or spleen displacement and movement of the ipsilateral hemidiaphragm, and identify the most reliably imaged regions of the diaphragm.

A multi-centre randomised controlled trial investigating the use of caloric management for the treatment of Refeeding Syndrome among critically ill patients
This NHMRC prospective study investigated whether an amino acid infusion protected the kidney from acute kidney injury during an ICU admission

CHEST (Crystalloid versus Hydroxyethyl Starch Trial)
A multi-centre randomised controlled trial of fluid resuscitation with starch (6% hydroxyethyl starch 130/0.4) compared to saline (0.9% sodium chloride) in intensive care patients on mortality.
EARLY PARENTERAL NUTRITION STUDY
This study determined the benefits of starting parenteral nutrition earlier than would normally be started under standard care, and whether early TPN improved survival measured at 60 days after commencement of treatment.
