About the service
The aim of this service is to educate and support participants to change their lifestyle in an effort to prevent or delay the onset of type 2 diabetes, heart disease and stroke.
Who can use the service?
This service is suitable for people who are interested in changing their lifestyle to improve their health into the future:
Discuss with your Gp if you feel this is an option for you and you are;
- Are aged 45 years and over and do not have diabetes
- Are Aboriginal Victorians aged 18 years and over who do not have diabetes
- Are adults aged 18 years and over who have a history of Gestational Diabetes Mellitus or Ischemic Heart Disease and have not had a diagnosis of diabetes.
