Adult Community Mental Health Service

The Adult Community Mental Health Service is known as the Community Liaison Early Intervention and Acute Recovery Service (CLEARS) which includes four specialist, multidisciplinary and integrated teams based on geographical areas that provide an Acute (previously known as the CAT team) and Recovery Service to people on the Mornington Peninsula.

The Service ensures that individuals referred to the service who are experiencing a serious mental health crisis or a relapse of a mental illness have easy and seamless access to treatment and support based on their needs.

All urgent or non-urgent referrals to Mental Health Service are through Mental Health Telephone Triage on 1300 792 977

Mental Health Clinicians support consumers to identify and work on their goals as well as providing a range of treatments that have been found to be helpful to others recovering from mental health problems. Family members, partners, and support persons are given information and are welcome to be involved in a consumer’s care (with consent).

Treatment Plans

Treatment plans are a mechanism for brief, active documentation about progress and treatment goals between Mental Health Clinicians and consumers. The treatment plan is focused on recovery and provides direction for treatment and care in the short term and longer term. Family members and other carers are encouraged to be actively involved in the planning process.

Focus of Care:

• Understanding the experience and working toward recovery

• Keeping safe

• Client-centred care and having information about treatment

• Effective medication

• Support with any drug and alcohol issues

• Developing a relapse prevention plan

• Learning ways of coping with stress

• Working on having a healthy lifestyle

Consumers are supported to have access to other services and specialists in order to assist them with resuming their interests, social activities and employment or study options as soon as possible.

Discharge planning

Discharge planning commences from the earliest phase of service delivery so that treatment and care remain directed to this objective. Discharge plans clearly nominate the consumers GP to be involved in their ongoing treatment, monitoring recovery and also relapse prevention.

 If  further help is needed from us after discharge, the consumer, family member and/or nominated person/s are encouraged to recontact us and we provide support as soon as possible.
