About Critical Care Liaison Service

Frankston Hospital’s ICU treats some of the Mornington Peninsula’s sickest patients. This year 807 patients were admitted to the 10 bed unit.

The first few days following a patient’s discharge from the Intensive Care Unit (ICU) can be stressful for the patient, their families and the nursing staff caring for them. There also is the risk of readmission to the ICU if preventable complications are not managed appropriately on the ward.

The Critical Care Liaison Nurse has been introduced to this year at Frankston Hospital to provide support to patients, families, and nurses on the wards that are caring for a patient who have recently been discharged from the ICU.

The Critical Care Liaison Nurses work with ward staff in the management of deteriorating patients, as well as following up every patient that is discharged from the ICU to the ward. They also provide advice and support about the care of complex patients and equipment as well as taking a lead role in MET calls and respond blues.

Regular communication between the nurses and the wards can quickly identify when a patient is deteriorating and provide specialist medical and nursing intervention which reduce the likelihood of admission into the ICU.

Families report that they are less anxious, knowing that even though their loved one is no longer in the ICU and are confident that their condition will still be closely monitored by critical care trained staff.
