About Peninsula Post Acute Care (PenPAC)
Peninsula Post Acute Care (PenPAC) provides short term assistance following discharge from public hospital or following presentation to the Emergency Department. Post Acute Care aims to prevent hospital readmission.
Post Acute Care purchases and coordinates services provided during the transition to community or longer term care. The most common services provided are community nursing, personal care, home care services and convelescent accommodation.
Referrals are normally made by hospital staff prior to discharge.
