Within the Social Work and Integrating Care Unit there are several teams which ensure the pathway along the continuum of care provides the patient and family with the highest quality care and information from admission to their discharge and beyond.

Social Work Services

Social Work Services are available at all sites of Peninsula Health.

Services include, assessment of patient needs to support discharge planning, advocacy, referral and support for carers and liaison with external community agencies where necessary to support future care planning.

Cultural and Linguistic Diversity

The Social Work department is committed to ensuring that patients from diverse cultural backgrounds and their families have access to the hospital services they need.
