About Transition Care

The Transition Care program provides short-term support and management for older people after a stay in hospital. Participants are provided with a care package of services as part of an ongoing but slower recovery process while options for their longer term care arrangements are considered.

The program consists of a multidisciplinary team who deliver their services both to individual client’s homes and to a residential care setting. The program is goal orientated and aims to assist with long term planning for up to 12 weeks.

Peninsula Health has 12 home based packages and access to 25 beds at Regis Shelton Manor in Frankston.

Transition Care is an initiative of the Commonwealth Department of Health and Ageing and the Victorian Department of Human Services.
