An application form, provided by Mt Eliza VA&EP, will need to be completed, together with a letter from a doctor certifying the disability and a health professional assessment to determine the appropriate type of aid.
About the Victorian Aids and Equipment Program (VAEP)
VA&EP is administered by the Disability Services of the Victorian Department of Human Services to help people, who have a long term or permanent disability, live independently in the community by providing them access to equipment, aids and home modifications.
