About the Aged Care Assessment Service
The role of the Aged Care Assessment Service is to determine the overall care needs of frail older people and to assist them to gain access to the most appropriate types of services.
The objective of the Aged Care Assessment Service is to:
- Provide an independent and multidimensional assessment of the care needs of frail elderly people
- Prevent premature or inappropriate admission to aged care homes and help frail older people live in the community, where that is their wish
- Facilitate access to the combination of services that best meets the needs of assessed clients
- Promote the coordination of aged care and other support services to improve the appropriateness and range of care services available to frail older people
- Provide information and education about aged care and contribute to aged care services planning.
