About the Aged Care Assessment Service
The role of the Aged Care Assessment Service is to assess and support the overall care needs of frail older people in all environments and to assist them to gain access to the most appropriate types of services.
The objective of the Aged Care Assessment Service is to:
- Provide an independent and multidimensional assessment of the care needs of frail elderly people
- Prevent premature or inappropriate admission to aged care homes and support frail older people live in the community for as long as possible, where it is their wish to do so.
- Facilitate access to the combination of services that best meet the needs of assessed clients
- Promote the coordination of aged care and other support services to improve the appropriateness and range of care services available to frail older people
- Provide information and education about aged care and contribute to aged care services planning.
