About ACCESS
ACCESS is an innovative referral service that provides a single point of entry into Peninsula Health Community and Continuing Care programs and services.
The service provides quick and easy access for consumers, carers and all community members to Community & Continuing Care programs and services via one phone number.
Callers speak with a trained health professional who will provide an initial assessment of the client’s individual needs at the point of service entry. Once all necessary information has been collected clients are then referred to a Peninsula Health service/program appropriate to their needs.

- Aged Care Assessments (MEACAS)
- Community Rehabilitation Centres (CRC’s) – Frankston and Rosebud
- Cognitive, Dementia and Memory Service (CDAMS)
- Continence Clinic
- Falls Prevention Service
- Domiciliary Care
- Personal Assistance Call Services (MEPACS)
- Aids and Equipment
- Agestrong
- Tai Chi
- Stronger for Longer
- Post Acute Care
- Carer Respite
