What is the ROSS Team?
ROSS (Residential Outreach Support Service) is a dynamic multidisciplinary service that aims to support residents of high and low level care facilities. ROSS provides clinical support, education and resources, as well as promoting communication between GPs, Peninsula Health and the Residential Care sector.

REHABILLITATION
Peninsula Health provides inpatient and outpatient rehabillitation services.
Peninsula Health Sub- Acute Assessment Service (SAAS)
Do you have a patient that may need inpatient rehabilitation?
The SAAS provides a point of contact for GP’s and other agencies with patients that may require inpatient rehabilitation. This multidisciplinary service, involving Social Workers, Nurses, Geriatricians and Rehabilitation Consultants, operates at Frankston Hospital from Monday-Friday and can triage patients and arrange for sub-acute admission if required.
The SAAS can arrange for admission to any of the following;
- Rosebud Community Rehabilitation Services
- Golf Links Rd Centre
- Mornington Centre
Referrals to this service should include,
- Current medical issues and context of the clinical situation
- Clear statement about the goals of rehabilitation
- Relevant Pathology or Radiology results
Referring GP’s will be contact by the SAAS team in regards to the outcome of the referral.
