Patients Own Medication
Patients are encouraged to bring in their own medication to assist in accurate medication reconcilliation on admission. Any medication belonging to a patient is labelled and locked away during the inpatient stay, but is returned to the patient at discharge. If regular medication has been stopped during the admission, the pharmacist will talk to the patient about what they would like done with the medication that is no longer required.
GP’s can play a large part in assisting accurate medication reconcilliation by completing accurate referrals with a current medication list.
Pharmacy Updates
Electronic Pharmacy Updates are companions to the Electronic Discharge Summaries sent from Peninsula Health. The purpose of the updates is to provide GP’s with an accurate discharge medication list. They are completed when a pharmacist sees an error in the medication list that has already been sent to the GP in the discharge summary. If you receive a Pharmacy Update, please refer to it for the discharge medication list.

This form is used when GPs are organising referrals for transfusion. The PSM is a skilled nurse who liaises with the General Medical Unit (Peri-operative) under whom your patient will be admitted.
