GP referral form to Patient Safety Manager (PSM) This form is used when GPs are organising referrals for transfusion. The PSM is a skilled nurse who liaises with the General Medical Unit (Peri-operative) under whom your patient will be admitted.

Hospital in the Home Blood Transfusion Service for residential care residents
Patients that reside in residential care institutions that require blood transfusions for chronic anaemia can now be referred to Hospital in the Home (HITH) directly, removing the need for Ambulance/non urgent transport, avoiding waiting times and disruption for the resident.
Patients must be referred to the HITH Physician by a Medical Practitioner. The referring doctor can contact HITH directly on 9784 7065 or 0416 123 315 during business hours or fax request on 9784 7242.
All referrals must include attachments of current general haematology results (these include B12, Folate, Iron Studies, FBE, 0ptional – Faecal Occult), and any information on current investigations or needed investigations.
A pre requisite for the use of this service is that the patient has received a blood transfusion in the past with no adverse reaction. If this is a first transfusion the patient must be referred to the hospital through the appropriate channels (see GP liaison web site).
If you consider your patient to be an emergency they must present as any other emergency, through the Emergency Department.
