NEW!! Go Live for new clinical information system at Frankston Hospital September 7th 2011
Peninsula Health, as a Lead Agency for the roll-out of the Department of Health’s ‘HealthSMART’ electronic clinical information system, will be going ‘live’ at Frankston Hospital on September 7th 2011. The first stage went live at Rosebud Hospital on June 15th 2011. HealthSMART incorporates electronic ordering of pathology and radiology, results viewing, discharge summaries and discharge prescriptions. General Practitioners with patients admitted to Rosebud Hospital after June 15th and Frankston Hosptial after September 7th will be receiving this new style discharge summary. Peninsula Health’s sub-acute sites will go ‘live’ in November.

Obstetric Discharge Summaries
Discharge summaries for routine births are faxed to GPs as well as given directly to women on discharge to take to the GP at their next visit. Complicated births or those with significant medical interventions are sent via fax or electronic transmission to the nominated GP. Enquiries about obstetric discharge summaries can be made to the ward clerk on 9784 7450 or Health Information Services (HIS) on 9784 7446.

Radiology Reports Direct to GPs via HealthLink
Peninsula Health Department of Radiology has begun utilising HealthLink to distribute Radiology Reports electronically to local GPs. Contact HealthLink on 1800 125 036 to obtain a starter kit, then contact the the Clinical Systems Unit on Ph. 9788 1225 or email Clover@phcn.vic.gov.au
Discharge Medications / Pharmacy
Peninsula Health has pharmacy departments at Frankston Hospital, The Mornington Centre, Golf Links Rd and Rosebud Hospital. You can contact the pharmacy on 9784 7602 to discuss your patient’s medications on discharge.
Patients Own Medication
Patients are encouraged to bring in their own medication to assist in accurate medication reconcilliation on admission. Any medication belonging to a patient is labelled and locked away during the inpatient stay, but is returned to the patient at discharge. If regular medication has been stopped during the admission, the pharmacist will talk to the patient about what they would like done with the medication that is no longer required.
GP’s can play a large part in assisting accurate medication reconcilliation by completing accurate referrals with a current medication list.
